If your business is exempt from sales tax on purchases made for resale, select your state below to download the official resale/sales-tax-exemption certificate. Complete the form and return it to EHR — do not file it with the state.
You must hold a valid sales-tax permit/registration number in the applicable state to claim a resale exemption. A few states issue or validate certificates online rather than through a downloadable form; those states link to the official state portal instead of a PDF.
